COVID-19 MTK Mandatory Screening Reminder

Reminder we cannot let participants into the facility unless they have completed the online screening.
Please remember to complete your online screening 2 hours before EVERY ice. 
Please make sure you are completing the form the same day you are on the ice. 
Please make sure you are selecting the correcting arena and time (should be PM - not AM)
